IMPORTANT CLINIC UPDATES - JANUARY 2021
MASKS MUST BE WORN
In order to protect all patients, staff and physicians during IN-OFFICE clinic appointments, we are requesting that ALL patients >2 years old and their support person(s) should wear a NON-MEDICAL MASK during their entire office visit - whether that is one purchased, home-made, or simply a bandana. Ideally three cloth layers is best. We thank you for your cooperation.

IN-CLINIC APPOINTMENTS
Although we continue to offer in-office appointments during the COVID-19 pandemic, for the safety of all our patients, our staff, our physicians and our families, we would like to kindly remind you the following:
1. Please wait in your vehicle and call us on arrival. We will advise you when it is safe to come in.
2. Please ensure you are wearing your mask.
3. Every patient will be screened with a questionnaire and a temperature on arrival.
At this time we are NOT allowing family members to accompany patients to their appointments to minimize transmission risk of COVID-19. Only infants and children <16 years old can be accompanied by a parent.

TELEPHONE APPOINTMENTS
If a patient is uncomfortable coming to clinic OR they are in self-isolation due to possible exposure to COVID-19 or a pending a swab result, then the appointment will be converted to a 10 MINUTE telephone appointment.

PRESCRIPTION REFILLS
We strongly encourage all patients to book an (in clinic/virtual) appointment for prescription refills. This allows the physician to ensure that it is appropriate and required for your continued medical care.
If a request is made for a refill on the telephone or via the pharmacy (without an appointment) this will incur a $10 fee. This fee will need to be paid upfront and does not include narcotics, benzodiazepines or ANY triplicate prescriptions. This refill is simply to bridge you to your next scheduled clinic appointment.
Please kindly allow ample time and pre-book your appointment in clinic so that you do not incur the $10 fee.
